Despite these improvements, some healthy tissue may still be affected during treatment. When radiation is delivered to the chest, one potential complication is radiation pneumonitis, an inflammatory condition that affects the lungs.
Radiation pneumonitis can range from mild to severe. While many patients recover completely with appropriate treatment, others may develop significant breathing problems that require medication, oxygen therapy, or hospitalization. Recognizing the early warning signs and seeking prompt medical attention can greatly improve outcomes and reduce the risk of permanent lung damage.

What Is Radiation Pneumonitis?
Radiation pneumonitis is inflammation of the lung tissue caused by radiation therapy. Unlike infections such as bacterial pneumonia, radiation pneumonitis is not contagious and is not caused by bacteria or viruses. Instead, it occurs when healthy lung tissue becomes inflamed after being exposed to radiation.
The condition most commonly develops after radiation treatment for cancers involving or near the chest, including:
- Lung cancer
- Breast cancer
- Esophageal cancer
- Lymphoma
- Thymic tumors
- Other cancers require radiation to the chest
Not every patient receiving chest radiation will develop pneumonitis. - Wheezing

- Blood work to help rule out infection or other conditions

Treatment for Radiation Pneumonitis

Corticosteroids
For patients with significant symptoms, corticosteroids such as prednisone are the mainstay of treatment. These medications reduce inflammation in the lungs and are often prescribed for several weeks or months, with the dose gradually tapered under medical supervision.
It is important not to stop steroids suddenly unless instructed by your healthcare provider.
Oxygen Therapy
Patients with low oxygen levels may require supplemental oxygen, either temporarily during recovery or, in some cases, for a longer period if lung damage is more extensive.
Inhaled Medications

- Stay well hydrated.
- Eat a balanced diet with enough protein to support healing.
- Avoid smoking and secondhand smoke.
- Limit exposure to dust, chemical fumes, and other respiratory irritants when possible.
- Practice deep breathing exercises if recommended by your healthcare team.
- Report any worsening symptoms immediately.
Never ignore increasing shortness of breath or assume it is simply part of recovering from cancer treatment.
Can Radiation Pneumonitis Be Prevented?
Although radiation pneumonitis cannot always be prevented, advances in radiation oncology have greatly reduced the risk.
Modern techniques such as:
- Intensity-Modulated Radiation Therapy (IMRT)
- Image-Guided Radiation Therapy (IGRT)
- Respiratory gating
- Deep Inspiration Breath Hold (DIBH) for selected breast cancer patients
- Three-dimensional treatment planning allow radiation oncologists to better protect healthy lung tissue while delivering effective treatment to the tumor.
Even with these technologies, some risk remains because every patient’s anatomy, cancer location, and treatment plan are unique.
